In brief: if you have a doctor/nurse/magic healer character and you wanted them to get work related to their field, you're welcome to sign them up for the Transport clinic. This is a Transport-run, Transport-staffed facility, separate administratively and location-wise from the Initiative's own hospital.
That being said, the supplies and equipment are provided by the Initiative, and at the clinic's outset, Initiative NPCs provided training to the clinicians in using their newfangled future-dystopia handwaved machinery. Characters who don't want the Initiative prying or poking at them might want to seek care at the Transport clinic, but characters who don't want to partake of any Initiative-provided amenities or services won't find this to be an ideologically pure solution.
